Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Case Manager to coordinate interdisciplinary care, review inpatient records for appropriate utilization, and manage discharge planning in a dynamic hospital setting.

Qualifications include licensure in the state of the hospital and current BLS certification. Three to five years of inpatient experience is preferred, with a focus on coordinating across disciplines to optimize patient outcomes and payer relationships.

Qualifications

• Education and Training: Licensure in the state where the hospital resides is preferred.
• Professional disciplines of the designated case manager may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker.
• Current BLS certification required.

Responsibilities

• The Case Manager coordinates care across disciplines for a designated patient population and reviews inpatient records to ensure appropriate utilization and admission decisions.
• Implements discharge planning and coordinates with payors and the interdisciplinary team to achieve desired financial and quality outcomes.
