PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Case Manager to coordinate interdisciplinary patient care and discharge planning. You will review inpatient records to ensure appropriate utilization of hospital resources and collaborate with the care team to achieve quality outcomes.

Qualified candidates have 3–5 years of inpatient experience and state licensure; BLS certification is required.

Qualifications

• Education and Training: Licensure in the state where the hospital resides.
• Experience: Three (3) to five (5) years inpatient experience; BLS certification required.

Responsabilités

• Coordinate interdisciplinary patient care and assess discharge needs.
• Review inpatient records to ensure appropriate level of care and discharge planning.
• Maintain payor relationships and collaborate with the care team.
