Foundation Communities in Austin provides supportive housing and seeks a dedicated Supportive Housing Case Manager to deliver intensive case management to residents across South Austin locations. The role focuses on housing stability, health access, and personal growth within a permanentsupportive housing setting.

You will work with interdisciplinary teams to support residents’ well-being, financial management, education and employment services, and community engagement.

Qualifications

• At least one year of intensive case management experience for adults experiencing homelessness, exiting homelessness, or at risk of homelessness.
• Experience in mental health/substance use and crisis intervention.
• Master of Social Work recommended.
• Ability to communicate with people from diverse backgrounds.

Responsibilities

• Conduct intensive case management and care coordination to increase residents’ access to and engagement in mental and primary health care, substance use treatment, financial management, and overall self-sufficiency.
• Assess and diffuse crisis situations using de-escalation techniques and document incidents and outcomes.
• Develop strategies to build relationships with residents and connect them to resources and learning opportunities.
• Collaborate with Property Management to support safety and well-being of the community.
• Document delivered services in accordance with grant requirements and FC documentation systems.
• Meet target dates to document services and support housing retention and income stability.
