Responsibilities

• Provide field-based case management focused on housing stabilization, budgeting and life skills.
• Guide clients through housing search, applications and lease signing.
• Maintain HMIS documentation and quarterly voucher reports.
• Coordinate with partners to link education, health and support services.
• Maintain monthly contact with participants and track progress.
