Job summary

Foundation Communities is seeking a Case Manager for the Cardinal Point program in Austin, TX. This full-time role oversees a 10–20 household caseload under the Children’s HOME Initiative, coordinating services and supporting residents to navigate the 2Gen program and achieve housing stability.

Responsibilities include case management, goal setting, referrals, and collaboration with staff across departments.

Qualifications

• Bachelor’s degree in social work, counseling, psychology, or related field.
• Minimum of two years of trauma-responsive services experience with diverse and/or unhoused populations.
• Ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Manage a caseload of residents identified for the Children’s HOME Initiative with referrals as needed.
• Meet regularly with families to identify needs, set goals, and evaluate progress toward stability.
• Advocate for client self-determination; apply crisis intervention and de-escalation when necessary.
• Coordinate access to services from external social service providers and therapeutic referrals.
• Coordinate educational opportunities to improve participant skills.
• Collaborate with a multidisciplinary team and property management for housing stability.
• Ensure compliance with program and property guidelines and document all activities.
