Job summary

Foundation Communities is seeking a Case Manager for The Loretta in Austin, TX. You will oversee a caseload of 10–20 families in the Children’s HOME Initiative, coordinating services with managers, learning center staff, and resident services teams to break cycles of poverty.

You’ll advocate for clients, provide trauma-informed support, and help navigate 2Gen programs, while coordinating education and housing resources within a supportive, multidisciplinary team.

Qualifications

• Bachelor’s degree in social work, counseling, psychology, or related field with knowledge of family dynamics, poverty and homelessness, mental health and substance abuse.
• Minimum of two years of experience providing trauma-responsive services to diverse or unhoused populations.
• Ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Manage a caseload of residents in the Children’s HOME Initiative, with referrals and case management support.
• Meet regularly with families to identify needs, set goals, and evaluate progress toward stability.
• Advocate for client needs, provide crisis intervention and de-escalation when necessary.
• Coordinate access to resources from other social service providers and submit referrals for therapeutic services.
• Coordinate educational opportunities to improve residents' skills.
• Collaborate with a multidisciplinary team and Foundation Communities programs.
• Ensure compliance with program and property guidelines; address housing issues with property management.
• Maintain documentation per agency guidelines and attend required meetings and trainings.
• Support resident councils and outreach; some weekend hours may be involved.
