Job summary

SAMMinistries in San Antonio, TX is seeking a Supportive Housing Case Manager to deliver field-based, direct client services to families and individuals experiencing homelessness. You will help participants obtain and maintain permanent housing using a Section 8 Voucher, focusing on stabilization at program entry and linking to education, employment, and health resources.

Qualifications

• Bachelor’s degree preferred in social work or behavioral science.
• At least 2 years of case management experience.
• Experience with housing and case management techniques, trauma-informed care, housing first, motivational interviewing, crisis intervention, harm reduction, and critical time intervention.

Responsibilities

• Provide field-based case management focused on independent living skills and housing stabilization.
• Guide clients through housing search, application, lease signing, move-in, and stabilization.
• Develop client-driven service plans addressing short- and long-term goals.
• Apply Trauma-Informed Care, Housing First, Motivational Interviewing, Harm Reduction, and CTI.
• Collaborate with Opportunity Home San Antonio to provide Set-Aside Vouchers.
• Assist clients through voucher eligibility appointments and processes.
• Maintain housing stability plans and assist clients in obtaining permanent housing.
• Maintain HMIS documentation and quarterly reports.
• Attend meetings and adhere to governance and ethics.
