Job summary

Penny Lane Centers is seeking a Facilitator to serve as the Intensive Care Coordinator for FSP, IFCCS, and Wraparound referred families. The role leads planning, interfaces with county workers, the Interagency Screening Committee, and the court system, and supports team communication and plan updates in a community-based setting.

The Facilitator must be able to lead meetings, solve problems creatively, and work collaboratively.

Qualifications

• Master's degree (M.A.) with at least 1 year of experience working with children and/or families.
• Or Bachelor's degree (B.A.) with at least 2 years of experience working with children and/or families.
• 3 years direct service experience in an Intensive Services program if lacking a college degree.
• Volunteer and/or personal experience as a child in foster care will be considered.

Responsibilities

• Lead planning as the team lead for FSP, IFCCS, and Wraparound referred families.
• Interface with county workers, the Interagency Screening Committee, and the court system.
• Attend all Child and Family Team (CFT) meetings in the agency, homes, or community.
• Lead team meetings, facilitate communication, and update Plans of Care/Client Treatment Plans.
• Ensure program adherence to County contracts and established protocols.
