Child & Family Services, Inc. seeks a Care Coordinator to organize services per the Individual Care Plan (ICP) and collaborate with the CPT and CSA program team.

The role emphasizes home-based assessments, safety planning, and ongoing contact with families and youth to ensure needs are met. Minimum education includes a Master’s or Bachelor’s degree; MA CANS certification is required, along with a valid MA driver’s license and weekend/evening availability.

Formación

• Care Coordinators must have a Master's or Bachelor's degree and relevant experience with child/family systems.
• Must hold a valid Massachusetts driver's license and own vehicle.
• Certified in the Massachusetts CANS.

Responsabilidades

• Conduct comprehensive home-based assessments using CANS and other tools.
• Develop and implement youth- and family-centered ICP with family input.
• Coordinate services with state agencies, schools, and providers.
• Provide family education, advocacy, and support, and monitor ICP updates.
• Offer after-hours phone support and arrange home visit services as needed.
