Job summary

Alternative Family Services in San Mateo, CA seeks an Intensive Care Coordination/Support Counselor to work in the field with the treatment team. You will help families identify a strong care team and organize Child and Family Team Meetings, while providing in-home, school, or community interventions aligned with therapy goals.

The role involves documenting sessions, collaterals, and meetings, with some evenings and weekends required.

Qualifications

• Background clearances including fingerprint and government checks.
• Bachelor’s degree in social science; or associate with relevant experience.
• 40 hours ISFC training, plus 20 hours ongoing within 12 months.
• CPR/First Aid certification.
• Valid CA driver’s license and insured vehicle.
• Fluent reading/writing in English; meet linguistic needs of target population.
• Clear verbal and written communication; work with diverse teams.
• Ability to work with minimal supervision and strong time management.
• High integrity and strong work ethic.

Responsibilities

• Provide support services to child and foster family per treatment philosophy and approach.
• Deliver a variety of interventions: coaching, mentoring, skill building, tutoring, feedback.
• Provide emergency response; evenings and weekends may be required.
• Ensure safety and well-being of the foster child at all times.
• Transport children to appointments/settings using a personal vehicle.
• Submit written documentation to CPS workers.
• Ensure compliance with HIPAA privacy practices.
• Adhere to attendance and punctuality policy.
• Assume other duties as assigned by supervisor.
