• MDHHS ICCW trainings are required annually, and initial 3-day training in Lansing, MI.

Responsibilities

• Facilitate family preservation and support services for ICCW-eligible families.
• Conduct strengths and psychosocial assessments.
• Develop family service plans with the Child and Family Team.
• Provide or arrange outreach crisis intervention and case management services.
• Link and advocate with other agencies to access health and human services.
• Support social functioning at home, school, and in the community.
• Attend required trainings and team meetings.
• Perform other duties as assigned.
