Job summary

Shineforth Treatment Foster Care is seeking an Intensive Care Coordinator to support youth and families in the community. You will build teams, set goals, plan transitions, and monitor progress to keep youths safely in their homes with family and community supports.

Responsibilities include attending trainings, applying High Fidelity Wraparound principles, facilitating team meetings, and writing plans of care with FAPT reports. Bachelor’s degree and 2 years of clinical experience required.

Qualifications

• Bachelor’s degree with at least 2 years of direct clinical experience providing children’s mental health services.
• Completion of “High Fidelity Wraparound” training preferred.
• Experience partnering with families and community resources is desirable.

Responsibilities

• Work with youth and families to create teams, develop goals, plan and coordinate transitions.
• Monitor progress to ensure that a youth can maintain in the community with their family.
• Attend mandatory county and state trainings on ICC and High Fidelity Wraparound.
• Facilitate all youth and family team meetings and ensure participation of all members.
• Develop, revise, and distribute team plans of care and FAPT reports as needed.
• Conduct initial and discharge assessments including strengths and needs of youth and family.
