Job summary

Oceans Healthcare, L.L.C. seeks an Outpatient Navigator to support patients moving from inpatient care to Oceans Outpatient Program. Under supervision, you coordinate admissions and ensure documentation is complete for discharge to IOP.

You will contact families, coordinate with case management, and follow up after the first two weeks in the outpatient program, ensuring smooth transitions and adherence to regulations.

Qualifications

• Experience coordinating inpatient to outpatient transitions.
• Strong communication with patients and families.
• Knowledge of discharge planning processes.

Responsibilities

• Educates inpatient groups about IOP services and benefits.
• Facilitate admit process for patients stepping down to IOP.
• Ensure new admit documentation is provided timely.
• Escort patients at discharge to IOP group.
• Coordinate with families for therapy session participation.
• Coordinate with case management for discharge planning.
• Respond to patient and family inquiries about outpatient program.
• Conduct follow ups after first 2 weeks in outpatient program.
• Communicate concerns to IOP director promptly.
• Perform other duties as assigned.
