Foundation Communities in Austin, TX is seeking a Supportive Housing Case Manager for South Austin locations. This full-time role provides intensive case management to residents in permanent supportive housing, helping them access health care, financial services, education, and employment opportunities.

Responsibilities include crisis assessment and outreach, collaboration with Property Management, and documentation for grants.

Qualifications

• At least one year of intensive case management experience with adults experiencing homelessness or at risk.
• At least one year of experience in mental health/substance use and crisis intervention.
• Strong independence, problem solving, and communication with diverse backgrounds.
• Ability to work within a blended property management model and engage residents respectfully.

Responsibilities

• Conduct intensive case management and care coordination with an interdisciplinary team.
• Assess and de-escalate crises; document incidents and outcomes.
• Develop outreach and activities to engage residents and build community.
• Collaborate with Property Management to enforce policies and ensure safety.
• Document services per grant requirements and FC systems.
