Job summary

Wisconsin Community Services, Inc. is seeking a Housing Case Manager to support individuals and households facing homelessness across Milwaukee County. The role focuses on housing stability, tenancy support, and resource linkage while working in a hybrid office and field setting.

You will build trust, coordinate with landlords and community partners, and implement person-centered, recovery-oriented approaches to help clients achieve independence and stable housing.

Qualifications

• Bachelor's degree preferred or an associate degree with at least one year of homeless-service experience.
• Ability to meet background checks, driving record, and drug screening requirements.
• Valid driver's license and access to a vehicle.

Responsibilities

• Support housing stability by helping clients maintain housing and secure tenancy.
• Develop and update an Individualized Service Plan every six months.
• Complete at least four community-based contacts per client monthly, including two home visits.
• Coordinate with landlords and partners to address concerns and prevent evictions.
• Maintain thorough documentation and case notes in electronic health records.
