Job summary

Walker's Point Youth & Family Center is seeking a Case Manager for the Youth Homelessness Demonstration Program in Milwaukee. The role provides intensive housing and case management to young adults facing homelessness or housing instability, including rapid rehousing, housing navigation, and connections to resources.

The successful candidate will work collaboratively with internal and community partners, conduct home visits, assist with moves, and ensure safe, stable housing for participants

Qualifications

• Bachelor’s degree in social work, human services, psychology, sociology, or related field.
• Three years of relevant experience in social services, housing, homelessness, youth services, or related field.
• Lived experience valued as part of qualifications.

Responsibilities

• Provide intensive housing and case management to youth/young adults experiencing homelessness.
• Maintain caseload, conduct weekly home visits, support housing stability and goal progress.
• Assist with move-ins, lease-up, and tenancy support.
• Coordinate with partners and landlords to facilitate services and referrals.
• Maintain participant records in HMIS and other required databases.
