Job summary

Blue Kite Wellness is seeking an Intensive Home-Based Clinician II to deliver home-based clinical services for children, youth, and families in the Illinois Pathways to Success program. The role involves assessment-informed planning, evidence-informed practice, and collaboration with Care Coordinators and School partners.

Qualified candidates will hold QMHP/LPHA credentials, have a master’s degree in a relevant field, and at least two years of related experience.

Qualifications

• Master’s degree in social work, counseling, psychology, or a related behavioral health field.
• Two years of progressively responsible behavioral health experience with children and families.
• Bilingual in Spanish and English.

Responsibilities

• Provide Intensive Home-Based Clinical services per approved service plans and Illinois Pathways requirements.
• Develop and adjust individualized clinical interventions using IM+CANS and assessments.
• Deliver individual, family, caregiver, and cross-system therapy to reduce symptoms and improve family functioning.
