Job summary

Blue Kite Wellness is seeking an Intensive Home-Based Clinician II to deliver in-home and community-based behavioral health services for children, youth, and families under the Illinois Pathways to Success program, with telehealth when allowed.

The role develops assessment-informed plans, provides therapy to reduce symptoms and improve family functioning, and guides IHBS staff, collaborating with caregivers, schools, and care coordinators.

Qualifications

• Meets QMHP or LPHA requirements per Illinois guidelines.
• Master’s degree in social work, counseling, psychology, or related field.
• At least two years of behavioral health experience with children and families.
• Strong engagement, treatment planning, crisis prevention, and documentation skills.
• Ability to work independently in home/community settings with a multidisciplinary team.

Responsibilities

• Provide Intensive Home-Based Clinical services to eligible children, youth, and families in accordance with service plans and Illinois Pathways.
• Use IM+CANS, FBA, clinical observations, family input, and Team recommendations to develop and adjust interventions.
• Deliver individual, family, caregiver, and cross-system therapies to reduce symptoms and improve functioning.
• Strengthen caregiver capacity, family communication, problem-solving, coping, and emotional regulation.
• Provide clinical direction and consultation to IHBS staff and review progress.
• Collaborate with caregivers, Care Coordinators, schools, and crisis-response providers.
• Support crisis prevention, safety planning, and post-crisis follow-up within scope.
• Monitor client progress and adjust services as needed.
• Complete timely, accurate documentation per Blue Kite Wellness and Illinois HFS requirements.
• Participate in supervision, case consultation, and ongoing professional development.
