Blue Kite Wellness is seeking an Intensive Home-Based Clinician to support children, youth, and families under the Illinois Pathways to Success program. This field-based hybrid role travels to client homes and communities, with telehealth when permitted, delivering in-home behavioral health services and coordinating with the care team.

Qualifications

• Meets current Illinois QMHP/LPHA requirements.
• Master’s degree in social work, counseling, psychology, or related field.
• Experience or willingness to work in home- and community-based settings.
• Ability to collaborate with care teams and schools.
• Strong documentation and communication skills.
• Reliable transportation for client visits.

Responsabilités

• Provide intensive home-based clinical services to children, youth, and families.
• Develop and update individualized intervention plans using MAP/IS Pathways guidance.
• Deliver individual, family, and caregiver interventions to reduce symptoms.
• Guide IHBS staff with supervision and consultation as needed.
• Coordinate with Care Coordinators, schools, and other providers.
• Maintain timely clinical documentation and participate in supervision.
