Job summary

Blue Kite Wellness seeks an Intensive Home-Based Clinician to deliver clinical services in clients’ homes and communities under the Illinois Pathways to Success program. The role emphasizes partnership with families, caregivers, and Care Coordinators to build intervention plans and reduce symptoms while maintaining safety and home stability.

Flexibility for travel in local areas is required, with telehealth supported where allowed.

Qualifications

• Meets Illinois QMHP or LPHA requirements.
• Master’s degree in social work, counseling, psychology, or related field.
• Entry-level to early-career clinicians welcome with proper qualifications.
• Strong assessment, treatment planning, and care coordination skills.

Responsibilities

• Provide Intensive Home-Based clinical services to children, youth, and families in the Illinois Pathways to Success program.
• Develop and update individualized clinical interventions using MAP and IM+CANS data.
• Deliver individual, family, and caregiver interventions for symptom reduction and family functioning.
• Provide supervision/consultation to IHBS staff as you gain proficiency.
• Collaborate with caregivers, Care Coordinators, schools, and other providers.
• Support crisis prevention, safety planning, and post-crisis follow-up.
• Complete timely clinical documentation and participate in supervision and training.
