Job summary

Blue Kite Wellness is seeking an Intensive Home-Based Clinician (IHBC) to support children, youth, and families in the Illinois Pathways to Success program. The role delivers individualized, short-term behavioral health services in clients’ homes and communities, with telehealth when permitted, under MAP and HFS-approved approaches.

You’ll collaborate with Care Coordinators, Care Team members, and schools, providing client-centered interventions, supervision, and documentation to ensure safety

Qualifications

• Meets current Illinois QMHP or LPHA requirements and applicable supervision/credentialing.
• Master’s degree in social work, counseling, psychology, or related field.
• Ability to work independently in home and community settings while staying connected with clinical team.
• Strong knowledge of clinical assessment, treatment planning, crisis prevention, de-escalation, and documentation.

Responsibilities

• Provide Intensive Home-Based Clinical services for children, youth, and families in the Illinois Pathways to Success program.
• Develop intervention plans using MAP, IM+CANS, and team input.
• Deliver individual, family, and caregiver interventions to reduce symptoms and improve family functioning.
• Support caregivers in communication, problem-solving, and emotional-regulation skills.
• Provide clinical direction to IHBS staff and participate in supervision and training.
