Job summary

Linc Housing Corporation seeks an Intensive Case Manager for the ICMS team in Los Angeles. You will coordinate services for formerly homeless individuals with chronic conditions and co-occurring disorders, reporting to the Senior Case Manager and enabling housing stability.

Responsibilities include collaborations with VA and housing authorities, intake and plan development, crisis interventions, and facilitating life-skills programs.

Qualifications

• Psychosocial assessment and intervention skills required.
• MS Word and Excel required; PowerPoint preferred.
• HIPAA compliance and privacy knowledge.
• Ability to think critically and organize complex care plans.

Responsibilities

• Work with VA and Housing Authorities to coordinate services.
• Assist with lease-up paperwork and program enrollment.
• Develop rapport and connect residents to mental health and housing services.
• Provide strength-based case management and service coordination.
• Lead and participate in community collaborations and meetings.
• Maintain accurate documentation and reports.
