Job summary

Paycom seeks a Behavioral Health Consultant to support patients within a Primary Care Behavioral Health model in Phoenix, AZ. You will assess psychosocial factors, provide brief interventions, and coordinate care with a multidisciplinary team.

Responsibilities include screening with PHQ-9/GAD-7, developing treatment plans, and connecting clients to community resources. Flexibility for varied schedules may apply; bilingual English/Spanish preferred.

Qualifications

• Experience with psychosocial assessment and treatment planning.
• Must be (at least) 21 years old.
• Ability to work flexibly and collaboratively with a patient care team in all settings.
• Excellent oral, written, and interpersonal communication skills.
• Knowledge of electronic health records systems and computer applications (MS Office, PowerPoint, Chrome).
• Ability to collaborate with professionals from various social services, healthcare, and government agencies.

Responsibilities

• Provide brief interventions and short-term counseling using appropriate modalities for depression, anxiety, trauma, grief, addiction, and related concerns.
• Conduct comprehensive assessments identifying psychosocial factors impacting health management.
• Utilize screening tools (e.g., PHQ-9, GAD-7, DAST, AUDIT) to determine care levels.
• Develop patient-centered treatment plans with a multidisciplinary team.
• Monitor progress toward goals and adjust plans as needed.
• Deliver interventions individually or in groups; crisis intervention and safety planning as required.
• Document all encounters in the electronic health record per policy.
• Coordinate referrals to community services and address homelessness-related needs.
