Job summary

Family Health Services Corporation seeks a Behavioral Health Consultant to join outpatient clinics. You will provide brief clinical interventions to chronic illness patients within a primary care framework and support care management and referrals.

Responsibilities include triage, case management, and collaboration with care teams, with emphasis on evidence-based techniques and timely documentation.

Qualifications

• Master’s degree in Social Work or Counseling.
• Current Idaho license in good standing or license eligible.

Responsibilities

• Provide brief clinical intervention to a caseload within established guidelines.
• Collaborate with primary care teams to coordinate care and referrals.
• Maintain documentation of services per agency formats.
• Participate in performance improvement activities and team meetings.
• Engage in advocacy efforts supporting the agency’s mission.
