Job summary

Qualifications

• Master’s degree in Counseling, Social Work or Doctorate in Psychology.
• Arizona LPC/LCSW or Psychologist license preferred.
• Two years of healthcare experience.
• BLS certification and strong documentation practice.
• Bilingual English/Spanish preferred.

Responsibilities

• Provide brief interventions to promote functional improvement and reduce barriers to care.
• Identify psychosocial factors influencing health and treatment outcomes.
• Coordinate with multidisciplinary team to develop patient-centered plans.
• Document encounters in the electronic health record with accuracy and timeliness.
• Refer and connect patients to community services addressing homelessness and related needs.
• Deliver crisis intervention, safety planning and rapid assessment as needed.
