Job summary

Circle-the-Cit in Phoenix seeks a Behavioral Health Consultant to address psychosocial factors affecting health outcomes and to provide brief interventions within a Primary Care Behavioral Health model.

You will coordinate with a multidisciplinary team, assess needs, create patient-centered plans, and connect patients to community resources addressing homelessness-related challenges. BLS and applicable licensure are required.

Qualifications

• Must be (at least) 21 years old.
• Arizona BBHE license: LPC, LCSW or AZ Board of Psychologist Examiners—other disciplines not considered.
• Two years of experience in the health care field.
• Basic Life Support (BLS) certification required.

Responsibilities

• Provides services within the integrated Primary Care Behavioral Health model.
• Performs comprehensive psychosocial assessments to identify factors affecting health management.
• Develops patient-centered treatment plans with measurable goals.
• Delivers brief interventions and short-term counseling (individual or group).
• Connects patients to community resources and services addressing homelessness-related needs.
• Maintains timely documentation and adheres to confidentiality policies.
• Participates in meetings, trainings, and continuous professional development.
