Job summary

Qualifications

• Two years of social work, human service or related professional experience.
• Ability to communicate clearly and concisely in both oral and written form.
• Bilingual in English and Spanish preferred.

Responsibilities

• Meet with patients to facilitate access to social services and coordinate delivery of resources.
• Provide education, referrals and follow-up for social service referrals with care teams.
• Act as navigator among service providers and community resources.
• Identify patients needing insurance or medication coverage and assist with enrollment.
• Attend team and staff meetings and maintain updated knowledge of resources.
• Adhere to program policies and regulatory requirements.
