Job summary

Mental Health Resource Center, Inc. is seeking a System of Care (SOC) Navigator to centralize referrals and coordinate access to services across MHRC programs and community resources.

The role focuses on uninsured and underinsured individuals, ensuring linkage within seven days of discharge and providing education about CSC Central Receiving System. It also emphasizes outreach to high utilizers to prevent crisis episodes.

Qualifications

• Bachelor’s degree in Social Work or related Human Services field required.
• At least 1 year of experience in human services or behavioral health required.
• Experience with adults with mental illness preferred.
• Strong communication, interpersonal, organizational, and problem-solving skills required.
• Proficiency with Microsoft Office, Outlook, and internet-based resources.
• Proficiency with the RBHS/MHRC EHR within 3 months of employment.

Responsibilities

• Care coordination and referral management with MHRC and community referrals.
• Triage, track, and follow up referrals to ensure timely linkage to care.
• Facilitate warm handoffs with behavioral health, medical, social services, housing, and partners.
• Maintain knowledge of MHRC programs, intake pathways, and resources.
• Support discharge planning and post-discharge follow-up to ensure linkage.
