• Demonstrates proficiency with computers and standard office technology.
• Valid driver's license.
• Ability to complete Kansas Community Health Worker Core Competency Training within 12 months of hire date.
• Ability to complete required Accountable Food Is Medicine training within 12 months of hire date.

Responsibilities

• Develop relationships with patients and families to identify health-related social needs.
• Connect patients with healthcare and community resources to address barriers.
• Provide education and support to patients and families.
• Coordinate referrals and care plans and follow up on identified barriers.
