Qualifications

• Strong communication skills, both oral and written.
• Crisis intervention skills and ability to work in a multi-disciplinary team.
• Bilingual English and Spanish preferred.

Responsibilities

• Assess patient needs for social services and coordinate access to care.
• Provide referrals and education on social services, including insurance and housing resources.
• Act as navigator among hospital and community resources and advocate for patients and families.
• Identify patients needing medication coverage and assist with enrollment when needed.
• Attend team meetings and participate in ongoing training to stay current on resources.
