Qualifications

• Minimum Education Required: High School Diploma or Equivalent
• Minimum Experience Required: 2 years in a healthcare setting
• Certification Requirement: BH Level: BHPP
• Preferred Certification: CCMA and Phlebotomy Certification

Responsibilities

• Greets members and prepares them for appointments in a timely, professional manner
• Obtains and documents vitals prior to each appointment
• Updates and verifies demographic information and preferred pharmacy details
• Assists with member flow to reduce wait times and support on-time provider schedules
• Completes appointment reminder calls, texts, and outreach
• Works outreach lists to reduce no-shows and close gaps in the schedule
• Coordinates re-schedules and follow-up appointments as directed
• Identifies urgent concerns and escalates per protocol
• Receives, tracks, and submits medication refill requests
• Collects information for prior authorization requests and submits timely requests
• Maintains tracking logs for timely follow up
• Sends and tracks referrals for labs, imaging, and specialty providers
• Coordinates lab orders and results routing to providers for review
• Communicates scheduling instructions and next steps to members
• Supports continuity of care by coordinating information with external providers
• Enters and updates information in the EHR accurately
• Maintains organized records and correspondence per policy
• Supports supply checks and clinic readiness tasks
• Participates in team huddles and communicates barriers and operational needs
