Job summary

Intermountain Centers is seeking a Care Coordinator to provide integrated case management for members, coordinating assessment, planning, implementation, and monitoring within the electronic health record. The role focuses on whole-health management and collaboration with an interdisciplinary team.

The ideal candidate will have a behavioral health background, strong documentation skills, and the ability to advocate for member access to resources.

Qualifications

• Bachelor’s degree in behavioral health, social work, or related field.
• High School diploma with 2 years behavioral health related experience.

Responsibilities

• Observe members, listen to concerns, and record the observations.
• Document and/or report any areas of concerns related to the member’s behaviors/interactions in clinical records per policy.
• Support the agency’s mission, goals, and management decisions.
• Monitor the health and safety of members, the signs and symptoms of their mental illness, reporting any concerns to the appropriate staff in accordance with reporting policies.
• Exhibit professionalism and positive role modeling for members, peers, and outside groups/visitors.
• Read, record and interpret information, including assessments, measures, and diagnostic criteria.
• Provide services and coordination based on member needs/requests.
• Work effectively with the member’s support system and clinical/therapeutic groups.
• Act as an advocate and liaison for member’s access to resources to support service plan goals.
• Participate in supervision as required by licensing, agency, funding source or as requested by supervisor.
• May be required to transport members in personal or company vehicles.
• Identify appropriate providers through continuum of care and communicate with an interdisciplinary team.
• Work with primary care professionals to improve and maintain quality member care.
• Maintain and update member records, including assessments and treatment plans.
• Develop individual service plans that are time specific and action oriented.
• Complete progress notes using appropriate billing codes and meet engagement/unit expectations.
• Complete all required training.
• Explore community resources to seek alternative options.
• Monitor and evaluate the effectiveness of the individual service plan.
• Follow policies and HIPAA requirements regarding PHI protection and access.

Education
