Job summary

Qualifications

• Bachelor's degree in behavioral health or related field.
• Associate's degree in behavioral health with 2+ years related experience.
• High school diploma with 4+ years behavioral health related experience.

Responsibilities

• Observe members, listen to concerns, and record observations.
• Document and report concerns related to member behaviors in clinical records.
• Support agency's mission, goals, and management decisions.
• Monitor health and safety of members and report concerns per policies.
• Maintain professionalism and act as a positive role model for members and peers.
• Read, record, and interpret information including assessments and criteria.
• Provide services and coordination based on member needs/requests.
• Work with member support systems and clinical/therapeutic groups.
• Advocate for member access to resources to meet service plans.
• Maintain and update member records and treatment plans.
• Develop individualized service plans that are time-specific and actionable.
• Complete progress notes using appropriate billing codes.
• Complete required trainings per licensing and funding sources.
• Explore community resources for alternative options.
• Monitor and evaluate effectiveness of service plans.
• Adhere to HIPAA policies and protect PHI access.
