Responsibilities

• Observe members and record observations.
• Document and report concerns in clinical records per policy.
• Monitor members’ health and safety, and report concerns as required.
• Maintain member records, assessments and treatment plans.
• Develop time-specific individual service plans with action steps.
• Complete progress notes using covered service codes and meet engagement expectations.
• Coordinate with interdisciplinary teams to improve member care.
• Assist in accessing community resources to support the service plan.
