Job summary

Qualifications

• Bachelor’s degree or 2 years related experience with equivalent training.

Responsibilities

• Observe members, listen to concerns, and record observations.
• Document and report concerns related to member behavior in clinical records.
• Support agency mission, goals, and management decisions.
• Monitor health and safety of members and report concerns per policies.
• Maintain member records and develop time-specific service plans.
• Collaborate with primary care and behavioral health teams to improve care.
