Qualifikationen

• Bachelor’s degree or higher in behavioral health or related field.
• Associate’s degree plus 2 years related experience.
• High school diploma with 4+ years behavioral health related experience.

Aufgaben

• Observe members and record observations.
• Document and report concerns in clinical records per policy.
• Support agency’s mission, goals, and decisions.
• Monitor health and safety of members and report concerns.
• Exhibit professionalism and serve as a positive role model.
• Read, record, and interpret information including assessments and criteria.
• Provide services and coordination based on member needs/requests.
• Collaborate with member support system and groups.
• Advocate for member access to resources to support service plan goals.
• Participate in supervision as required by licensing and funding sources.
• May transport members in personal or company vehicles.
• Identify providers and facilities across continuum of care.

Ausbildung

Bachelor’s degree in behavioral health or related field

Associate’s degree in behavioral health/related field + 2 years experience

High school diploma + 4 years behavioral health experience
