Responsibilities

• Observe members, listen to concerns, and record observations.
• Document concerns in clinical records per policy.
• Monitor health and safety of members and report concerns.
• Provide services and coordination based on member needs.
• Coordinate with primary care/therapists to support care plans.
• Maintain and update member records and treatment plans.
• Complete progress notes using billing codes and deadlines.
• Identify providers and resources across the continuum of care.
• Follow HIPAA policies and maintain PHI security.
• Possess mobility for de-escalation and related duties.
