Job summary

Southwest Behavioral & Health Services in Payson, AZ seeks an Integrated High Needs Care Coordinator to join our interdisciplinary team. You will provide integrated case management to clients with complex medical and behavioral health needs, advocating for access to services and supporting wellness self-management.

You will lead family-centered care planning, coordinate with providers, and maintain thorough documentation while staying compliant with HIPAA.

Qualifications

• Minimum age 22
• High school diploma or GED with 5 years integrated/behavioral health experience
• Bachelor’s degree in related field with 1+ year integrated/behavioral health experience preferred
• Valid Arizona driver’s license and pass motor vehicle screening
• Fingerprint clearance or eligible to obtain upon hire
• 3.3 weeks PTO in first year with accruals

Responsibilities

• Facilitate family and youth ISP development from assessments and team input
• Provide information and support on treatment planning and community resources
• Coordinate care and linkages to services with youth/families
• Maintain accurate case documentation and progress notes
• Communicate with clients, providers, and staff to ensure coordinated care
• Ensure confidentiality per HIPAA and conduct after-hours on-call as needed
