Job summary

Southwest Behavioral & Health Services is seeking an Integrated High Needs Coordinator I for the Broadway Outpatient Clinic in Phoenix. You will work with an interdisciplinary team to provide integrated case management and advocacy to improve wellness and access to services, including crisis intervention support and care coordination.

This full-time role requires a Bachelor's degree in psychology or related field, 1–2 years of behavioral health experience, and a valid Arizona driver’s license.

Qualifications

• Bachelor’s degree required in Psychology or related field.
• 1–2 years behavioral health experience required.
• Knowledge of DSM, crisis intervention and community resources.

Responsibilities

• Facilitate CFT meetings for assigned clients.
• Develop ISPs from assessments and team input.
• Educate and support clients on treatment planning and coordination of care.
• Maintain accurate documentation of contact and progress.
• Ensure confidentiality per HIPAA guidelines.
