Job summary

Qualifications

• Master's degree in social work or related field (accredited).
• Current state licensure as LCSW, LMFT, CMHC/LPC/LCPC, LMFT, or Psychologist.
• Excellent written and verbal communication skills.
• Computer skills (email, word processing).

Responsibilities

• Diagnoses and treats mental, emotional, and behavioral disorders; modifies care plans as patient status changes.
• Refers patients to community services, treatment programs, support groups, psychiatrists, or other providers to improve well-being.
• Works with a multi-disciplinary team to develop discharge plans for continuity of care.
• Organizes daily work by assessing patient needs and priorities in areas of responsibility.
• Performs psychosocial assessments and provides crisis, bereavement, and behavioral health interventions as needed.
