Job summary

Holon Health in New Mexico seeks a compassionate Case Manager to monitor client progress, coordinate with Justice Liaisons and correctional sites, and support integrated care.

This hybrid role is 80 percent remote, 20 percent in-person across Springer, Los Lunas, Grants; you’ll schedule appointments, verify insurance, assist with community resources, and guide clients through our platform.

Qualifications

• Strong organizational and relationship-building skills across health plan liaisons and facility managers.
• Excellent communication and interpersonal skills with diverse populations, non-judgmental.
• Ability to conduct concise, supportive phone conversations.
• Knowledge of community, justice system, and community connectedness.
• Understanding of healthcare systems and medical insurance.
• Ability to provide emotional support and motivation to clients.
• Bilingual English/Spanish preferred.
• Experience with EHR/EMR systems.
• Confidentiality and discretion essential.

Responsibilities

• Review client activity including test results, logins, lesson completion, and attendance.
• Monitor progress against individualized treatment plans.
• Coordinate with clinical team for interventions when deviations occur.
• Address barriers to engagement with clients.
• Verify insurance coverage and eligibility.
• Coordinate scheduling with Justice Liaisons and facilities for in-person/telehealth.
• Arrange provider appointments and reschedule as needed.
• Support clinical team with client follow-up.
• Help clients access housing, transportation, employment resources.
• Coordinate referrals with external agencies.
• Troubleshoot issues with proprietary platform; guide users.
• Respond to client inquiries; provide ongoing encouragement.
