Qualifications

• Utah resident and county residency required.
• Must hold Utah Case Management Certification or SSW License.
• Excellent relationship-building and communication skills.
• Detail oriented, self-motivated, independent and a team player.
• Ability to travel to client sites and visit facilities.

Responsibilities

• Managing clients services- weekly face-to-face contact with clients, assessing clients needs, coordinating housing services.
• Documentation- completing, updating, submitting, and compiling necessary documentation for clients.
• Regularly meeting and communicating with clients at their facility
• Communicating with other case managers to coordinate clients’ services.
• Work with clients to find and maintain housing
• Build and maintain relationships with clients and community partners
• Regularly attend staff meetings and company trainings
• Able to use your own devices
• Able to travel to visit clients in the listed county
