Job summary

Holon Health is seeking a compassionate Case Manager to join a collaborative care team in a hybrid role based in New Mexico, with responsibilities spanning proactive client progress monitoring, treatment adherence, and coordination with health-plan liaisons and facility managers. The role supports in-person and telehealth appointments across Springer, Los Lunas, and Grants.

Ideal candidates are organized, culturally sensitive, and skilled in coordinating across diverse teams.

Qualifications

• Strong organizational and relationship-building skills across health plan Justice Liaisons and facility managers.
• Excellent communication and interpersonal skills in diverse settings; judgment-free and culturally sensitive.
• Strong time management and multitasking abilities.
• Proficiency with healthcare systems, medical insurance basics.
• Willingness to provide emotional support and motivation to clients.
• Bilingual English/Spanish preferred.

Responsibilities

• Review client activity within the program, including test results, logins, and appointment adherence.
• Monitor progress against individualized treatment plans.
• Coordinate in-person and telehealth appointments at correctional facilities.
• Verify insurance coverage and eligibility for treatment services.
• Schedule and manage referrals with external agencies and providers.
• Provide support to clinical team for client follow-up and resource access.
• Assist clients with housing, transportation, and employment resources.
