Job summary

Center for Life Management (CLM) provides outpatient mental health services in the Derry, NH area. The Integrated Care Case Manager role supports care planning, resource identification and coordination to improve mental and physical health outcomes.

Work is largely remote via telehealth with some in-person visits. Applicants should have a Bachelor's degree (or relevant Associates with experience) and demonstrate clinical judgment, documentation, and teamwork.

Qualifications

• Bachelor’s degree preferred; Associate’s degree with direct service experience may qualify.
• Ability to exercise sound clinical judgement in routine, urgent, and emergent situations.
• Proficient computer skills and ability to document efficiently in clinical work.

Responsibilities

• Provide care planning and assessment to create individualized plans.
• Assist clients with applying for/maintaining medical coverage for access to care.
• Coordinate internal and external supports and services for wrap-around care.
• Provide counseling and Functional Support Services in home or community settings.
