Job summary

The ideal candidate will have a high school education and four years of experience in behavioral health or a bachelor's degree with related experience. We prefer bilingual candidates (English/Spanish).
Qualifications
- 4+ years of experience in behavioral health and SMI or 1 year with a Bachelor's degree.
- Valid AZ Driver's License and compliance with driving requirements.
- Bilingual preferred.
Responsibilities
- Engage and support service participants with the Integrated Care Team.
- Develop and manage Integrated Service Plans (ISPs).
- Coordinate care across healthcare providers.
- Monitor progress towards recovery and wellness goals.
- Support program needs with additional duties.
