Job summary

Qualifications
- High School degree required or Bachelor's in a related field with one year of experience.
- Minimum four years of experience in behavioral health and SMI or equivalent.
- Arizona valid Driver’s License and safe driving record.
Responsibilities
- Coordinate care with service participants and healthcare providers.
- Develop and maintain the Integrated Service Plan (ISP) for each member.
- Document progress in managing chronic medical and psychiatric conditions.
