Qualifications

• High School degree required or Bachelor's in a related field with one year of experience.
• Minimum four years of experience in behavioral health and SMI or equivalent.
• Arizona valid Driver’s License and safe driving record.

Responsibilities

• Coordinate care with service participants and healthcare providers.
• Develop and maintain the Integrated Service Plan (ISP) for each member.
• Document progress in managing chronic medical and psychiatric conditions.
