Job summary

Native American Connections, Inc. in Phoenix, AZ is seeking a Case Manager to join our Full Time Clerical Behavioral Health/Patina Wellness team. The role coordinates care, completes intake assessments, facilitates groups, and connects clients to community resources.

Under the Clinical Supervisor, you will update treatment plans, document progress notes, and support services addressing social determinants of health. Strong communication and cultural competence are essential.

Qualifications

• Associates degree in behavioral health or related field with two years’ experience in substance use or mental health.
• High school diploma or GED with four years of relevant experience.
• Strong knowledge of community resources and referral sources.
• Valid Arizona driver’s license and ability to obtain Fingerprint Clearance Card.
• CPR/First Aid within 90 days of hire.

Responsibilities

• Complete initial intake/assessments and related paperwork in a timely manner.
• Facilitate health promotion, psychoeducational and skills training groups, and individual case management.
• Update treatment plans as needed.
• Complete daily clinical documentation including progress notes and intake/discharge paperwork.
• Assist with development of client cultural competencies.
• Address Social Determinants of Health needs and coordinate referrals (food, housing, transportation, benefits).
• Coordinate care among treatment teams and referral sources; participate in Adult Recovery Team meetings.
• Provide Front Desk coverage as needed.
• Support CLAS Standards and trauma-informed practices.
