Job summary

CareSource in Massachusetts is seeking an Integrated Care Clinical Manager - BH to lead complex care management for dually eligible enrollees. You will coordinate medical, behavioral, and social services, supervise the ICT, and drive health outcomes in home and community settings.

The role requires a Master’s in social work or mental health counseling with an independent license, strong communication skills, and the ability to travel within Massachusetts to visit members and providers.

Qualifications

• Master's degree in social work or mental health counseling with independent license is required.
• BH Clinician capable of serving complex medical, behavioral, and social needs.
• Experience in care coordination or case management with dual-eligible populations preferred.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage enrollees in homes and community settings to build effective care management relationships.
• Lead interdisciplinary care team to create holistic care plans addressing medical and social needs.
• Coordinate with providers and community resources to ensure seamless care transitions.
• Perform comprehensive assessments and follow-up to monitor enrollee status.
• Support access to housing, transportation, and social services while promoting preventive care.
