Family Health Center Of Kalamazoo seeks a Behavioral Health Therapist/Consultant to join our primary care team using the PCBH model. The role combines brief interventions and 1:1 therapy within a multidisciplinary setting to support whole-person health.

Responsibilities include screenings, assessments, crisis coordination, and collaboration with physicians and care teams. Requires Michigan licensure and experience in integrated care environments.

Qualifikationen

• Master’s degree in social work, Counseling, Counseling Psychology, or related behavioral health field.
• Current, active Michigan licensure: LMSW, LLMSW, LPC, or LLPC.
• Must be in good standing with the State of Michigan.
• Limited license candidates must be actively working toward full licensure and eligible for clinical supervision.

Aufgaben

• Conduct brief behavioral health screenings for adults, adolescents, and children.
• Perform comprehensive clinical assessments to determine diagnosis and appropriate level of care.
• Diagnosing and treating mental health and substance use disorders within scope of licensure.
• Deliver brief, solution-focused, evidence-based interventions.
• Provide individual, family, couples, and/or group therapy as assigned.
• Coordinate care with internal and external providers, with patient consent.
• Participate in multidisciplinary team meetings and case consultations.
