Job summary

Grace Health is seeking an Integrated Behavioral Health Consultant to deliver brief, evidence-informed interventions within our PCBH model in a primary care setting. You will work as an integrated member of the health care team to improve both physical and mental well-being for patients across ages and families.

You will collaborate with medical staff, provide real-time feedback to PCPs, and document all encounters in the EHR.

Qualifications

• Master’s degree or higher in a behavioral health field (MSW, psychology, counseling, or PhD/PsyD).
• Licensed in Michigan as LMSW, LPC, LP, or equivalent; must obtain full licensure if on limited license.
• Minimum 2 years of combined pre- and post-degree behavioral health or integrated primary care experience; PCBH/IBH experience preferred.

Responsibilities

• Provide brief, solution-focused behavioral health interventions within PCBH model.
• Collaborate with the primary care team to develop patient-centered care plans.
• Conduct same-day warm hand-offs and scheduled visits for referred patients.
• Document all encounters in the EHR per organizational standards.
• Participate in quality improvement and interdisciplinary team activities.
